Tube passengers trapped as carriage filled with smoke and fumes
the driver was not aware what was happening and the doors remained closed.

Two minutes after the train stopped some passengers escaped through the inter-car doors at the end of each carriage and then climbed over the flexible barriers to reach the platform. Others tried to open the double doors but they could not be opened enough to let most people off.

During this time, the report continues, the driver was trying to find out what had happened "and the train doors adjacent to the platform remain closed."

It was two and a half minutes before station staff reached the platform and opened one pair of doors from the outside. It was another minute before the driver opened all the doors on the platform side.

Bob Crow, the RMT union leader, said: "The events provide graphic and compelling evidence as to why trains and stations need to be fully staffed at all times..."
